Instructions & Guidelines for Filling the Application Form

General Instructions

1. All entries in the form must be written clearly in block letters, by the candidate, in his/her own handwriting.

2. Incomplete applications will not be accepted.

3. Every part/column of the form must be filled. If something is not applicable, please write “N/A”.

4. Providing false or incorrect information will lead to cancellation of admission.

5. Copies of all required documents must be attached with the form.

6. No credit will be given for documents submitted after the deadline.

7. Original documents must be presented at the time of admission (they will be returned after certified copies are made).

8. The completed application form must be submitted to the concerned department or sent through a registered courier service.

9. The application must reach the department before the last date mentioned in the admission notice (advertised in newspapers, on the college website, or official social media).

Checklist – Please ensure the following are attached:

• Two (2) certified copies of the applicant’s CNIC.

• Two (2) certified copies of Father/Guardian’s CNIC.

• Four (4) passport-size photographs with a blue background.

• Two (2) sets of attested photocopies of all educational documents.

Declaration (To Be Signed by the Applicant)

I solemnly declare that:

1. I agree to follow all rules and regulations of the institution, including discipline and code of conduct.

2. I will pay all dues before the 10th of each month. If delayed, I agree to pay a fine of Rs. 50 per day.

3. All fees, fines, and other dues will be deposited in the designated bank branch, and the deposit slip (challan) will be submitted to the institution.

4. I understand that if I fail to pay my dues for more than two months, the institution has the right to cancel my admission.

5. I accept that admission and other fees are non-refundable in case of cancellation due to non-payment or disciplinary action.

6. I understand that the college has the right to cancel my admission and registration with the respective council if I am found guilty of immoral behavior or misconduct within the premises.

7. I confirm that all documents and certificates submitted are genuine. If found otherwise, my admission will be cancelled, and legal action may be taken against me.

8. I agree to undergo examination and evaluation by the college before registration with the respective council.

9. I understand that all fees once paid are non-refundable and non-transferable under all conditions.

10. I undertake to show good behavior at all times.

11. I agree to pay the full course fee (Rs. ___) according to the selected installment plan.

12. I undertake not to indulge in politics during my stay at the institution. Violation of this clause will result in expulsion without notice.

13. I am joining this course with the consent of my father/guardian, who accepts responsibility for my conduct.

14. I accept all current and future rules/regulations of KIHS as binding upon me.

15. I will immediately notify the institution in case of any change of address.

16. I have read and understood all the rules and regulations related to admission before signing this application.
